Vacation Rate Request Form

• This form must be filled out in order to receive the Vacation Rate (half tuition rate).
• Please make sure to give this notice at least two weeks prior to your vacation date, to ensure you are not charged your regular tuition rate.

Medicine Authorization Forms:

• These forms must be filled out in order to authorize Little Oak’s Staff to administer prescribed, or doctor approved, medicine to your child.

Ongoing Medicine Form

• This form is to be used for medicine that has an undetermined end date.

Temporary Medicine Form

• This form is to be used for temporary medicine that has an determined end date.

Service Termination Request

• This form is to be filled out when you plan to terminate childcare services with our center.
• This form is meant to protect you, the client, from paying for weeks of service you will not be using.
• Please make sure to give this notice at least three weeks prior to termination.
